What Is a Coronectomy?

A coronectomy is a surgical procedure in which the crown of a lower wisdom tooth is removed while the roots are intentionally left in place. It is an alternative procedure to full wisdom tooth removal.

It is most commonly performed when a lower wisdom tooth lies very close to the inferior alveolar nerve (IAN), the nerve that provides sensation to the lower lip, chin, and lower teeth.

The aim is to treat the problematic part of the tooth whilst reducing the risk of nerve injury.


Why Might a Coronectomy Be Recommended?

Most wisdom teeth can be removed completely. However, in some patients, X-rays or CT scans show that the roots are closely related to the inferior alveolar nerve.

The procedure is similar to a conventional wisdom tooth removal.

What Happens to the Roots Afterwards?

Following coronectomy, the roots often move slightly within the bone during healing. This process is known as root migration.

In most patients:

Occasionally the roots migrate towards the surface of the gum. If this occurs and symptoms develop, removal of the roots may be recommended. By this stage, the roots have usually moved away from the nerve, making later removal safer.
